| A facility shall immediately report every serious accident and/or incident to the attending physician, next-of-kin or responsible party, and the Department via telephone, email, or facsimile within twenty-four (24) hours of the serious accident and/or incident. |
A facility shall submit a written report of its investigation of every serious accident and/or incident to the Department within five (5) days of the serious accident and/or incident. |
In addition to the initial report as may be required by Subsection (B)Facilities shall submit a separate written investigation report for the following types of incidents within seven (7) business days from when the facility had reasonable cause to believe an incident occurred via the Department’s electronic reporting system or as otherwise determined by the Department.

- Patient Protection Events
- Care Management Events
- Environmental Events
