The Deputy area of Healthcare Quality offers a clear and efficient process for filing complaints against health care facilities — such as nursing homes, adult day care centers, etc. — and licensed service providers such as midwives, hearing aid specialists, etc. Your feedback is vital to maintaining high standards of care and safety in health care settings. State Accident/Incident Reporting for facilities

For all serious accidents or incidents (as defined by regulation) facilities must submit to DPH an initial report to Healthcare Quality within 24 hours of occurrence. Once investigated, facilities must submit a full report within 5 days.
