This application is to be used by all hospitals to file information about their emergency preparedness for hurricane evacuation and other disaster/emergency events, as required by Regulation.

Information provided in the Critical Data Sheet system will serve as the basis for the Governor's decision to allow an exemption to the Mandatory Medical Evacuation. Submission of the required information does not ensure exemption from the evacuation declaration.
